Waiver Form

Facility Entry Requirement: Signed Waiver Required

To enter Funtropolous, a completed waiver is required for every single person entering the building. This applies to all adults and children, even if you are not playing and only supervising.

Adults (18+): Must complete and sign their own waiver.
Minors (Under 18): Must be included on a waiver signed by their parent or legal guardian.
Spectators: Every non-playing adult must still have a signed waiver on file to enter.
